Clients | Please submit this form as a way to prepare me to hear from the person vetting you. I will only provide references if it has been 6 months or fewer, or up to three references since our last meeting. After that, I will inform providers that it has been a while or that you have asked me for multiple references and that you should have a more recent one by now.

Providers | Include as much information about the client as possible, (name, phone, email, date of last meeting), as well as your personal ad or website.
